CDCS Support Planning Services for BI, CAC, CADI, DD and EW waiver plans & Alternative Care programs
- Provide information about consumer directed community supports and provider options.
- Help develop and submit individual’s new and renewing community support plans and health and safety plans.
- Research and coordinate purchases and the delivery of services and supports for the individual.
- Implement, monitor and evaluate the approved Community Support Plan and budget on an ongoing basis.
- Create and submit modifications to the Community Support Plan as needed including revisions and addendums.
- Assist with recruiting, screening, and hiring, and training resources for support workers and respite.
- Provide information about community resources related to the Community Support Plan.
